Skilled Nursing Care

Skilled nursing means round-the-clock clinical oversight. Licensed clinical staff are on site at all hours, and a supervising physician directs your care plan. Whatever your medical needs, there’s always someone qualified, attentive, and ready.

Directed by your physician, our full team of nursing, therapy, social work and nutrition staff designs, implements and regularly reviews a care plan tailored to your specific needs.

Clinical Monitoring

A stroke can affect movement, speech, memory, and daily function in ways that vary enormously from person to person. Our integrated team of nurses and therapists builds a recovery plan around your specific situation, working with skill and patience to help you regain as much independence as possible.

Surgical wounds, pressure injuries, diabetic ulcers, and chronic wounds all require specialist assessment and a structured treatment plan. Our nursing team handles that with the clinical expertise and the attention to detail that proper wound healing demands.

Some treatments have to continue after a hospital stay ends, and coming in for them every day is exhausting. Our nurses administer intravenous antibiotics, fluids and other infusion therapies here, so you can carry on with your recovery without the travel and the waiting.

Recovering after a cardiac event or managing a long-term heart condition takes careful monitoring and steady, unhurried support. Our nursing team works with your cardiologist to keep medication, activity and diet on track, watching for the small changes that matter before they become setbacks.

Pain that is not properly controlled slows everything else down, including therapy. Our team assesses it honestly and often, combines medication with hands-on and therapeutic approaches, and adjusts the plan as your recovery changes rather than leaving you to manage on a schedule set weeks ago.

Dialysis happens here at Northwest, in our own on-site center. No ambulance at dawn and no ride across the city, just treatment where you already live, with people who already know you.

Discharge Support

Home, but not alone

When you’re ready to return home, our support continues. You’ll get a dedicated Discharge Concierge who is there to help you manage the transition and stay safe. They’ll answer your questions and address your concerns before they become problems, minimizing the risk of hospital readmission.
